We have optimized the structure of our Prevea360 provider network offering, adding a second comprehensive provider network option for State of Wisconsin Group Health Insurance Program members to select.
Prevea360 Health Plan and Mayo Clinic Health System are partnering to offer a new network option in western Wisconsin for State of Wisconsin Group Health Insurance Program members starting with the fall It’s Your Choice open enrollment period.
The network spans 14 counties in western and northwest Wisconsin:
If you prefer to get primary care in eastern Wisconsin, then you should enroll with the State of WI – Prevea360 East network option.

Both network options will be available as a Tier 1 option during open enrollment, which begins on Monday, Sept. 26, 2022, and runs through Friday, Oct. 21, 2022.

ACTION REQUIRED: If your desired network option is Prevea360 West and Mayo Clinic Health System, you must enroll for 2023, and select this network to ensure access to your network of choice.
If you are currently enrolled in Prevea360 and want to remain in this network, no action is needed. You will automatically remain in the newly named network - Prevea360 East.
Additionally, you should review your new ID card closely. The Prevea360 West and Mayo Clinic Health System network name on your ID card will read as Prevea360WMayoClinic. The Prevea360 East network will continue to appear on your ID card as Prevea360.
